Registration forms are available via the link below.  NEW players MUST include a copy of a certified birth certificate along with their notarized registration form and fees.  Incomplete/Un-notarized registrations, or registrations without full payment will not be accepted.  

Fees for the 2012 season are $240 for U13 and older (Competetive), and $140 for U12 and younger (Micro).  Players will be assigned to teams on a first come - first served basis.  Before a team will be registered, a team must have a coach, a referee and a complete roster.  Make checks payable to LYSC.  Send the completed and notarized form, payment and birth certificate (if it applies) to: (you may also register at the Registration Clinic on December 4th)
